Death is very sad thing that could happen to a member of a family.
Securing a death certificate facilitates whatever benefits or inheritance you can
get out of this sad occurrence. This legitimizes any transfer of properties or assets
one could leave behind. This is also required should the spouse left behind wants to
get married again.
Procedures for Registration of Death

Go to the Office
of the Municipal Health Officer and look
for the Nurse or Sanitary Inspector. They will be
the one to prepare for you the certificate
of death of your deceased loved one/relative. It
is advised that the death of the deceased
shall be registered within 1 month after
his death otherwise, additional
requirements will be provided for the
delayed registration of said death.
It is advised farther that the immediate
family member of the deceased will be the
one to personally file and provide the
necessary information. (For those
who died in Guinayangan Medicare
Community Hospital, the medicare hospital
personnel will be the one to prepare and
file for you the certificate of death of
your dead relative).
Please be ready to supply the following information:
- the complete name of
the deceased
- age, sex, and religion
- date and place of death
- citizenship, civil
status, occupation and
residence
- causes of death
- attendant at death
- corpse disposal method
Review every data
supplied on the prepared certificate of
death, giving primary importance on the
spelling of the deceased name, his sex,
and the date of his death.
Sign the
informant's portion of the certificate.
Return the
certificate to the Nurse or Sanitary
Inspector for necessary signatures of
the preparer and of the Municipal Health
Officer.
Proceed to the
Office of the Municipal Civil Registrar
and look for the Clerk, Registration
Officer or the Civil Registrar for the
necessary signatures and for filing.
If the deceased's
interment will be at the Municipal
Public Cemetery, pay the necessary
burial lot rental fee at the Office of
the Municipal Treasurer at the following
Schedule:
- Phase I of the Public
Cemetery
- P 25.00/3
years
- Phase II of the
Public Cemetery
- P
50.00/year
- Phase III of the
Public Cemetery
- P
200.00/year

Present your official receipt to the
Registration Officer or Municipal Civil
Registrar.
8. Secure the Original copy of the Certificate of Death.
9. No fees are being collected for registration of
death.
Procedures for securing a copy of Death Certificate

Go to the Office of the Municipal
Civil Registrar and look for the Clerk or
Registration Officer.
Please be ready to supply the following information:
- the name of the
deceased
- date of death
If the death of your relative is
indeed registered in this municipality,
the Clerk or Registration Officer will
prepare for you a certified copy of said
Death Certificate.
Proceed to the Office of the
Municipal Treasurer and pay P30.00 for a
copy of said certification.
Present your
official receipt to the Municipal Civil
Registrar.
Secure the certified copy
of the death certificate.
